Until now, the document that should be used to guide the preparation of waybills was Resolution of the State Statistics Committee of Russia No. 78 “On approval of unified forms of primary accounting documentation for recording the work of construction machines and mechanisms, work in road transport” (hereinafter referred to as Resolution of the State Statistics Committee of November 28, 1978 No. 78 ). However, this resolution applies only to legal entities.
In total, eight unified forms of waybills have been approved for legal entities of all forms of ownership carrying out activities related to the operation of construction machines or motor vehicles.
For recording the operation of construction machines and mechanisms:
- waybill for construction vehicle (form No. ESM-2).
For accounting of work in road transport:
- waybill for a passenger car (form No. 3);
- waybill for a special vehicle (form No. 3special);
- waybill for a passenger taxi (form No. 4);
- waybill for a truck (form No. 4-C);
- waybill for a truck (form No. 4-P);
- bus waybill (form No. 6);
- waybill for a non-public bus (special form No. 6).
Therefore, Order No. 68 of the Ministry of Transport of Russia was later issued, which applied to individual entrepreneurs. The forms of waybills given in it differed from those established by the State Statistics Committee for legal entities.
The following forms of travel documents have been approved for individual entrepreneurs:- waybill for a truck of an individual entrepreneur (form No. PG-1);
- waybill for an individual entrepreneur’s bus (form No. PA-1);
- waybill for a passenger car of an individual entrepreneur (form No. PL-1).
However, this document has lost force in accordance with clause 2 of Order No. 152 of the Ministry of Transport of Russia.
Order of the Ministry of Transport of Russia No. 152establishes only mandatory details and the procedure for filling out travel documents, but the rules contained in it apply to both legal entities and individual entrepreneurs.
Now the independently developed form must contain the following details provided:By Order of the Ministry of Transport of Russia No. 152:
- name and number;
- validity period of the waybill;
- information about the owner (holder) of the vehicle;
- information about the vehicle;
- information about the driver.
In addition, the waybill must record the date (day, month, year) and time (hours, minutes) of the driver’s pre-trip and post-trip medical examination. The indication of such details was not required either by the unified forms given in Resolution of the State Statistics Committee of Russia No. 78, orOrder of the Ministry of Transport of Russia No. 68. Legal entities must indicate the time of the examination next to the signature of the medical worker. By the way, this signature must be decrypted.

Organization
– fill in the name, address, telephone numbers of the organization at whose disposal the car is located (in our example, Avtolider LLC).
OKPO code
– fill in the code (All-Russian Classifier of Enterprises and Organizations) assigned to the organization.
car model
– fill in the make of the car (in our example, GAZ 3110). If the car is leased, then it is advisable to indicate the lease agreement (number, date and owner of the car), (for example: under lease agreement No. 256-A dated March 15, 2009 with Avtolider LLC).
State license plate
- state car number (in our example 082ee01).
Garage number
– to be filled in if the organization has several machines on its balance sheet. In this case, instead of the garage number, you can write down the inventory number of the car as a fixed asset.
Driver
– Full name of the driver (in our example, Ivanov A.R.).
Personnel Number
– employee personnel number assigned to him in the organization.
Certificate No.
- driver's driver's license number.
Class
– driver class.
License card, type, series
– currently licenses are issued only for commercial transportation of passengers of more than 8 people. Therefore, in the Waybill for a passenger car (Form No. 3), these fields are not filled in.
Available
– at whose disposal the car is presented (in our example, Avtolider LLC). If the car is leased or for the provision of services to a third party (legal or individual), then the name (surname) of this person is indicated. If a waybill is issued for a period of more than one day, then most likely it will be impossible to indicate a specific user, since there may be many such persons. In this case, it is advisable to indicate the name of the organization itself on this line ( in our example, Avtolider LLC), and the transcript of the surname of a particular user should be given on the reverse side of the sheet in the column “Signature of the person who used the car.”
Delivery address
– the address where the car should arrive for further work.
Time to leave the garage
– the time of leaving the garage is filled in. If the waybill is issued for several days, then it is advisable to indicate the date in addition to the departure time.
Dispatcher-contractor
– signature and full name of the dispatcher issuing the task to the driver (in our example, A.V. Soldatov).
Mechanic
– signature and full name of the mechanic releasing the car onto the line (in our example, Vasilyev S.N.). Before entering the line, the car must pass a test of the main units for operability. This check must be carried out by a mechanic. If all units are working normally, he signs the waybill.
In turn, the driver signs that he accepted the car in good condition.
Time to return to the garage
- the time for returning to the garage is filled in. If the waybill is issued for several days, then it is advisable to indicate the date in addition to the departure time.
Dispatcher-contractor
– signature and full name of the dispatcher confirming with his signature the time of arrival at the garage. The dispatcher also makes the necessary notes (lateness, waiting, downtime, visits to the garage, etc.) (in our example, A.V. Soldatov).
The driver handed over the car
– the signature and full name of the driver indicating that he handed over the car.
In the lower right part of the front side of the waybill, information on the movement of fuel is provided:
Fuel grade – fill in the brand of fuel the car is being fueled with.Issued according to the filling slip – the amount of fuel issued by the organization itself must be indicated (if it has fuel reserves or its own gas station). But in practice, as a rule, they enter data about refueling at third-party gas stations.
Remaining fuel upon departure – the amount of fuel in the vehicle’s tank before entering the line.
Remaining fuel upon return – the amount of fuel in the car’s tank after returning to the garage.
Normal consumption – the amount of fuel consumed by this vehicle on the basis of the Methodological Recommendations “Standards for the consumption of fuel and lubricants in road transport” (appendix to the order of the Ministry of Transport of Russia dated March 14, 2008 No. AM-23-r) (hereinafter referred to as the Methodological Recommendations). If your organization’s car is not included in the list presented in the Methodological Recommendations, then the standard can be determined by a technically similar vehicle (approximate dimensions, same engine size, similar technical characteristics, etc.).
The standard consumption is calculated using the following formula:
Normal consumption = Mileage X Fuel consumption rate / 100 .
Actual expense – the amount of fuel spent during work according to the waybill and is calculated as follows:
Actual expense = amount of fuel before departure + amount of fuel filled - amount of fuel upon return.
Speedometer reading when returning to the garage – speedometer reading upon return.
The mechanic records and confirms with a signature the time of return, the speedometer readings and the movement of fuel.
Saving - If the fuel consumption is actually less than the standard fuel consumption, then fill in this field.
Overspending - If the actual fuel consumption is greater than the standard fuel consumption, then fill in this field.

Filling the reverse side
It should be noted that both in the unified form and in the independently developed form of the waybill, information about the specific location of the vehicle is mandatory. 9. A waybill is issued for each vehicle used by a legal entity or individual entrepreneur for the transportation of goods, passengers and luggage by road and urban ground electric transport in urban, suburban and intercity communications.
10. The waybill is issued before the start of the trip, if the duration of the vehicle driver’s trip exceeds the duration of the shift (working day), or before the start of the first trip, if during the shift (working day) the vehicle driver makes one or more trips.
11. If during the validity period of the waybill the vehicle is used in shifts by several drivers, then it is allowed to issue several waybills for one vehicle separately for each driver.
12. The name of the waybill indicates the type of vehicle for which the waybill is issued (passenger car waybill, tram waybill, etc.). The waybill number is indicated in the heading section in chronological order in accordance with the numbering system adopted by the vehicle owner.

16.1. - state registration plate of a passenger car, truck, cargo trailer, cargo semi-trailer, bus, trolleybus;
- odometer readings (full kilometers) when the vehicle leaves the garage (depot) and enters the garage (depot);
- date (day, month, year) and time (hours, minutes) of the vehicle’s departure from the permanent parking place of the vehicle and its arrival at the specified parking lot.
Driver information includes:
- last name, first name, patronymic of the driver;
- date (day, month, year) and time (hours, minutes) of the pre-trip and post-trip medical examination of the driver.
Additional details may be placed on the waybill, taking into account the specifics of activities related to the transportation of goods, passengers and luggage by road or urban ground electric transport.
Procedure for filling out a waybill
A waybill is issued for each vehicle used by a legal entity or individual entrepreneur for the transportation of goods, passengers and luggage by road and urban ground electric transport in urban, suburban and intercity communications.
The waybill is issued for one day or a period not exceeding one month.
If during the validity period of the waybill the vehicle is used in shifts by several drivers, then it is allowed to issue several waybills for one vehicle separately for each driver.
The name of the waybill indicates the type of vehicle for which the waybill is issued (passenger car waybill, tram waybill, etc.). To become a professional driver, the ability to drive vehicles alone is not enough. Waybills: document structure and registration procedure
Despite the fact that waybills (PL) are not compiled according to a standard model, but in a way that is convenient for the employees themselves, they are considered official documents. Thanks to PL, accountants can perform primary accounting of the operation of vehicles (based on the data given in the document) and pay wages to drivers.
The preparation of waybills is entrusted to the dispatcher of the enterprise or to the one who acts as his representative. Usually the PL is issued in one copy, but if it is lost, a duplicate may be issued (such situations arise extremely rarely).
The validity period (legal force) of the document is limited to the duration of one work shift. If a company practices business trips lasting several days, then one waybill is issued for this entire period.
These are the main elements of the document:
- Waybill number.
- The date when the ticket was opened.
- Signature of the person who issued the document.
- Official seal of the company.
Purpose of waybill
Waybills are issued for an obvious purpose: to control the driver’s actions and record the wear and tear of the vehicle. The document reflects the mileage of the car, and this figure is compared with the planned indicator. Of course, there may be deviations, but in general the route should coincide as much as possible with the calculated one.
The legislation legalizes several main types of submarines, which are designed taking into account the characteristics of various vehicles (for cars, tractors, buses). If a driver provides inspectors with a ticket that incorrectly indicates the type of vehicle, he may be fined.
Types of waybills
In practice, the following three types of vouchers are most often used:
- N 4-s. This form is used in those companies that have chosen piecework wages for drivers. The salaries of such workers are calculated according to the average tariffs in force in this area.
- N 4-p. These are used to track the driver’s working time. The employee receives money for the period worked.
- N 4th. This form is used by those enterprises that engage in intercity transportation and delivery. Registration of waybills for such trips involves not only indicating more complete information, but also special forms (on the front side there is a red stripe and the inscription “intercity transportation”).
Vouchers for cars
A company car is a vehicle owned by a legal entity and its enterprise. The carrying capacity of such vehicles is usually small.
The company vehicle and the person driving it can be hired. In this case, it becomes mandatory to fill out a waybill. This document provides guarantees to both parties to the labor relationship: the employer is confident that the work has been completed in full, and the driver has written confirmation of his actions. A waybill is a kind of insurance that cannot be replaced by receipts or oral agreements.
Usually the following information is indicated on a passenger car ticket:
- The date on which the sheet was issued.
- Its number and series.
- Last name, first name and patronymic of the driver who will use the document.
- The date and time the work ends (end of the working day or return from a flight).
Of course, the waybill for a passenger car must be signed by the person who stores and accepts such documents. The driver also puts his signature. Before this (when receiving the voucher), he must check the correctness of the information.
Creation and storage of travel forms
Many enterprises prefer to use forms made in a printing house, others develop their own template and print these documents on printers. In any case, a blank waybill means that all copies are numbered, and the movement of completed documents (as well as damaged ones) is controlled. The forms must be stored in the company’s safe or in its warehouse; the responsible person issues the required number of vouchers when the need arises. The waybill, the form of which was damaged when filling out, is disposed of, and its number is entered into the list (this helps to avoid shortages during recalculation).
Who fills out the ticket and how?
Even if the car is not rented (it is used by the owner himself), its driver is given a permit. Before the start of each new flight, the dispatcher or the person performing his duties writes down information about the driver and the circumstances of the trip on the waybill.
The data that is usually included in the waybill for a passenger car must be accurate and reliable. The procedure for filling out vouchers is described in Resolution of the State Committee of the Russian Federation No. 78. Here is a list of eight basic forms developed for various:
- Construction transport.
- Passenger car.

- Taxi.

Filling out the waybill: front side

Fuel consumption
The volume of gasoline or gas consumed is also indicated on the voucher; there is a separate place on the form for this. Here you can find information about the brand of fuel, its quantity (what was issued and what you had to buy at gas stations), the remaining fuel in the tank before the trip, as well as the amount remaining after the end of the trip.
Before filling out the waybill, the dispatcher checks the planned gasoline (gas, diesel) consumption indicators, which are given in one of the orders of the Ministry of Transport of the Russian Federation. By comparing data on actual and planned consumption, the amount of savings or overexpenditure is calculated. The received data is recorded.
The instructions on how to fill out the waybill also require the signature of the health worker who released the driver on the flight.

The following information should be included here:
- The route along which the vehicle moved.
- A flight number that is assigned to each trip and recorded in the travel log.
- Places of departure and arrival.
- Actual trip mileage and driver's signature.
Other types of travel vouchers: tractor travel voucher (form 68)
In the case when a tractor is used to transport goods, its driver is given a permit. There are two forms of this document: form 68 and 412-APK.
The first option is used in several regions of Russia, geographically located near Ukraine. Farmers from these two countries are often united by economic relations, and it is easier for them to work using the same documents. In fact, such a tractor waybill is a Ukrainian form, but in Russian legislation there is a similar form.

Ticket for a bus driver
Like the other documents described above, the bus waybill is intended to record the time worked by drivers and is necessary for calculating their wages. This is one of the most important primary documents, which includes the following data:

If one bus is used by several drivers, and they work in shifts, then they make up the required number of vouchers (usually two). The bus waybill for the first driver will include the data recorded at the exit, and the second employee’s document will contain information when the vehicle arrives.

All waybills must be filled out carefully and without corrections. The employee drawing up the document is obliged to use only reliable and up-to-date information. Otherwise, the driver may be fined by inspection authorities.
